| Action2Care support people to live in their own home and enjoy an active life in their local community, achieving positive outcomes. Support is built around an individual, considering their likes and dislikes. Our approach to care and support is person-centred, needs-led embracing individuals’ personal goals and aspirations. We provide different levels of support, from just a couple of hours a day to full time care and live in care. We support people to live as independently as possible in their own home and lead meaningful and fulfilled lives. Our support workers are of the highest standard and fully trained to deliver person centred support. We ensure that the people who use our service and their families are at the centre of the care and support that is delivered, and welcome all feedback.
