Client Consultation: We are here to be your trained consultant. Ask us how we can help you utilize available resources such as: senior housing, social security, legal, care management, etc. Many times clients are surprised to hear that their Medicaid Service allows them to receive Home Care Services. In addition, our nurses provide FREE ASSESSMENTS on the first visit.
Non Medical Care: Preparing meal, infection control, companion sitter service, errands, shopping, outings, medical/social appointments, prescription/errands pick-up, Transportation and escorting, Fall prevention and risk reduction, home making assistance, linen changes and laundry, translation
Medical Care: Personal Care (bathing, toileting, etc), feeding, monitoring vital signs, mouth care, incontinence care and personal hygiene, monitor nutrition and fluid intake, mobility, ambulation, transfer exercise
Skilled Nursing Care: Medication review and management, wound care, colostomy care, hypertension and diabetes management, Indwelling Foley catheter care, Alzheimer's/dementia care, heart disease/ asthma/ and cancer care management, periodic care review.
Payment for Service: Long-Term Care Insurances, Medicaid ( Medicaid Waivers: SOURCE and CCSP are programs designated to help the elderly or people who have disabilities and need help to live in their home or community instead of an institution), and Private Pay
