I have two years of experience working in a supported living service for adults with additional needs and mental health issues. In this role, I provided personalised support both at home and in the community, ensuring person-centered care. My responsibilities included administering medication, maintaining accurate records, and adhering to all policies and procedures to safeguard those receiving support. Currently, I provide weekly one-on-one private support for a young adult with complex additional needs, requiring comprehensive assistance with their daily routine. My duties include full support with all aspects of personal care, meal assistance, medication administration, support with mobilisation, and use of individual equipment such as a stairlift and heart monitor, all while respecting and promoting their independence and choices. Additionally, I assist them in accessing community services and enhancing their daily life with enriching activities chosen based on their preferences or with their input where possible. Most recently I have supported an elderly lady with dementia on her palliative care journey. This role involved, companionship, housekeeping, running errands and personal care.
