I have worked as a Health Care Assistant and Support worker for over one year. I actively promote, encourage and support Clients to maximise independence. I have worked with different Clients/Patients with Dementia, Mental Health, Learning Disability, Vulnerable Adult, and Clients in Intensive Care to ensure the delivery of personal and practical assistance in accordance with the care plans and that all protocols are met. Also maintaining case notes to ensure that information is available for those people who are entitled and need to know.
