Do long distance caregivers miss out on a sense of satisfaction?

I often hear from the long distance caregivers I speak with that while they spend so much time and physical and emotional energy caring for their elderly loved ones (coordinating care, handling finances, calling doctors, making check-in calls, and just plain worrying!), they often do not experience the satisfaction or sense of accomplishment that local caregivers do. In fact, they often walk around with a sense of guilt thinking about all they're NOT doing! Is this true for you? Why do you think that is so?
