In my role as a Direct Support Professional, I provided care and support to individuals with diverse special needs, including autism, Down syndrome, and physical disabilities such as those requiring wheelchairs or feeding tubes. I was primarily assigned to a group of four young men, ages 18 to 45, though I was also responsible for up to six individuals depending on staffing needs. My daily responsibilities included assisting with personal care tasks such as bathing, repositioning clients, and providing support with mealtimes (including dinner and evening snacks). I conducted routine checks every two hours to ensure clients' comfort, safety, and hygiene, responding promptly to any immediate needs, including changing or repositioning. I also maintained detailed records by documenting each client's care activities at the end of my shift, including completing goal plan books, recording bowel movements, and entering data into the system.
