For the past 5 years I provided 24-hour in-home care for 2 high-risk Alzheimer's/Dementia patients, including activities of daily living (ADLs), physical care, memory care and emotional well-being using knowledge of safety and body mechanics. I was also responsible for monitoring and recording patients' behavior within patient progress notes for the family and care team with 0 incidents and a 100% satisfaction rating. I was also responsible for executing shopping, money management/bookkeeping, coordination of outside services (i.e. yard work), and driving patients to appointments. Maintained the household including cleaning, laundry, and meal preparation, ensuring patients followed their nutrition plan with an 90% success rate. Administered medication per patient care plan while observing for any adverse events or reactions requiring reporting to family and/or care team. Built a trusting relationship with 9 members of the patient's family through effective communication.
