Experience assisting Dementia, Alzheimer, Parkinson, Hospice care patients and elderly experiencing hearing/ vision impairment and/or mobility issues. Services provided: - Medication management and monitoring. - Transportation to doctor's appointments. - Patient advocacy and health record management, experienced in setting up telemedicine appointments and correspondence. - Administrative assistance (phone calls, email/mail correspondence, calendar management) - Light meal prepping and meal planning for proper nutrition and hydration. - Run errands (grocery shopping, regular or occasional transportation) - Exercise monitoring and assistance following physical therapy plan when needed. - Grooming, oral care, hygiene, and dressing. - Post-small surgery care and recovery.
