About Grace

I assists clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual. I encourage clients to do as much of their own care as possible. I assist with meal preparations, encourages residents to eat a proper diet and take adequate fluids while respecting the patient's right to free choice. I will work with other agency to supplement the clients care needs, report any changes in the clients physical condition and/or behavior, that may be impending the emotional condition of the client to the primary family member or professional care team. I offer medication reminders, I also assist with ADL and IDL's as needed. I will maintain a clean, safe, and orderly environment for the client in their home and I provide companion ship to the clients as well.
