About Chris

I've worked with elderly for over 20yrs. I focus on a plan of care that encourages and maximizes the independence of that individual and work along with the healthcare professionals to provide the appropriate treatment for illness and continuity of care. I perform daily assessment and look for changes in health status and identify changes that requires attention. I manage medication administration and refills also observe for effectiveness of treatment. I manage appointments and transport with attendance to physician offices. I provide the basic needs required for the client. Depending on the client's situation I adapt to perform tasks that allows comfort and flexibility for all parties involved in the continuity of care.
