About who needs care: Hospitalized four times in past two months for pneumonia, low sodium and pulmonary emboli. Will discharge from hospital in next couple of days, lives independently but now will need assistance. Ambulatory with walker, takes own meds, has upcoming physicians' appointments, very alert and oriented. About the care needs: Barb has her routine, loves to talk, loves sports and playing cards. Is diabetic but not insulin dependent on a blood thinner. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, and bathing / dressing.
