About who needs care: ffered his 2nd stroke and wants to return home with his wife (my mom) who has some beginning memory loss. He doesn't want to stay in the current family care home that he is currently at. He is working with PT and OT to try and make transfers by himself and toilet himself, but he isn't at that level yet. About the care needs: My dad is a retired Dairy Farmer and wants nothing more than to be back in his home but he is still not able to make transfers by himself so he can't toilet himself etc.
