About who needs care: has dementia after recent fall, has hospital bed but not bedridden. Requires significant assistance with transfers, some days she is unable to help with transfers at all. She spends most of day in wheelchair or recliner. PT/OT come several days a week About the care needs: Ability to physically help with transfers and patience/experience with patients with dementia required. Family member will be present in home in addition to patient to assist with transfers Services needed include: meal preparation, errands / shopping, light housekeeping, and mobility assistance.
