Hands-on Care Needed For Myself In Dallas

I need a hands-on care provider for myself, 65 yo male. I was diagnosed with spinal stenosis and multiple bulging discs which causes severe sciatica. This is very debilitating with excruciating pain. I've made remarkable recovery with physical therapy and chiropractic care. I would like to find someone who has some experience with therapy that could visit a couple of times a week. So clearly it would be ideal to have someone who has at least some physical therapy experience.
