Hands-on Care Needed For My Aunt In Nashville

I am looking for someone to start taking care of my aunt in East Nashville as soon as possible for about 3 hours every afternoon. She has high blood pressure (requires medication), and symptoms of dementia. She doesn't drive, and so currently depends on neighbors to bring her food and ensure she takes her medicine. Needs assistance getting to periodic medical appointments. Needs help preparing meals and making sure that she eats them. Some light housekeeping would be welcome but house has signs of long-term hoarding. About the care needs: I think she is also lonely, with little social contact, so someone who would be happy to chat and take an interest in her. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, and bathing / dressing.
