Weekly Care Needed For Mother With M.S. & Cognitive / Dementia-like Issues

I'm looking for hands-on care for my mother who just turned 69. She has had Multiple Sclerosis for over 20 years and there are dementia-like symptoms with memory problems. Basic cognitive function is significantly impaired and normal everyday tasks are very complicated. She currently has an incontinence issue where she needs to go see various doctors. We need someone who is able to provide assistance and transportation to appointments. She has a small dog that will need to have transportation to the vet at times as well. Help making phone calls. Chores around the house, ensuring she takes her medication etc. She does have mobility issues as well, she can walk but very slow and might need help in and out of vehicles.
