About who needs care: She recently had a hospital stay, which resulted in her moving into a nursing home for recovery. The OT and PT support her moving out of the nursing home with an approved set up of home health care and 24 hour care. She does have some mobility and can go to the bathroom on her own, with great effort, but she is a fall risk. She also needs help with other daily tasks. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, bathing / dressing, and mobility assistance.
