Hands-on Care Needed For My Father In Reading

I'm looking for hands-on care for my father who will be 80. He has MS and uses a wheelchair for mobility. Needs prompts for taking his medications. Help getting from recliner to wheelchair, not physically lifting, just guiding. Has to be watched that when transferring to the toilet that he's ok. We're looking for someone to provide companionship, give snacks and warm up meal plates. Needs prompts to keep drinking. Very easy going, friendly. Naps often in his recliner. Would also need someone comfortable with letting my little dog out periodically in our fenced in yard. At some times my adult son who is also disabled will be home on his computer but won't need any assistance. We are hoping for someone who is friendly, honest and has some experience with being with elderly people.
