Looking for LPN or RN to check on mom once a week to triage /identify any medical needs. Communicate with family and physician. If necessary initiate medical treatment with physician or urgent care setting. Available to receive requests for additional visits as necessary. About the care needs: Knowledgeable. Good listener and communicator. Caring. Reliable. Able to triage concerns.
