About who needs care: Hearing issues Lived alone for 12 years since loss of husband Active and independent Does all her own shopping and out with friends Had a partial hip replacement due to closed fracture 5/28 Discharged to twin pines 6/4 Want her home and rehabilitation will come in and occupational therapy weekly tbd days and times She can not be alone Family will cover 6pm-6 a.m. shift overnight Looking to have her engaged snd nap 2 xs day at maximum in bed not chair. No longer than 1 hour Walk with her Assist when needed mostly she can do walking and dressing with a little assistance for compression socks and shoes no direct bending to put on shoes and socks. Help with breakfast and lunch prep together No household chores just her engagement and safety Shower assistance will be a home health aid or daughter in law No male companion pleased she wont be comfortable Assistance to the bathroom and then assistance to come out of bathroom About the care needs: She loves to cook and read and play cards and talk about things A simple person who has a heart for elderly and is engaged in what is going on and willing to adapt to the changes as she becomes as independent as possible We will assess monthly the additional information and progress Services needed include: companionship.
