About who needs care: Nearly blind. Falls alot. Wears diaper but can be assisted to bathroom. Neuropathy in feet. Some chronic conditions he takes meds for. Needs to work on strength and movement daily. Wheeled around to activities and food area. More info to follow About the care needs: Out-going and loves to joke around, fun-loving, enjoys sarcasm CNA or LPN- Able to lift him if he falls down- Can hopefully prevent him from falling Help with seeing his food and plays activities in day room Doesn't mind changing his diapers. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, bathing / dressing, and mobility assistance.
