Hands-on Care Needed For My Father In Dayton

About who needs care: Has been in nursing home for the past 9 months and wants to come home to see if he can continue to live independently. He has low vision, left ear no hearing, mobility issues are present but improved. House has been remodeled so that he can use a wheelchair, but he is able to walk with walker. He is a diabetic and also goes to dialysis a few days a week. About the care needs: I'm in California and my dad is in Dayton. Family there has been helping him, but he wants more independence than they want to give him. He feels he should have a choice in the matter, so I am trying to get him in contact with someone that can assist with helping him in the home. Services needed include: meal preparation, errands / shopping, light housekeeping, companionship, feeding, bathing / dressing, and mobility assistance.
