History of multiple strokes, cardiovascular disease with open heart surgery 12/2023. Needs assist to shower and dress, uses cane or walker, poor balance, Reconditioned, easy fatigue. Receiving PT but not a lot of progress. About the care needs: A male who talks sports Services needed include: transportation, feeding, bathing / dressing, and mobility assistance.
