A 30-year-old female who is wheelchair-dependent requires assistance with daily tasks. She needs help with bathing and meal preparation. Twice a week, she attends therapy appointments via medical transportation, where a companion is required. Her weekly schedule includes 7 days of support some broken into two shifts: AM : Help with dressing, breakfast, and preparing a packed meal (prepared the night before), followed by assistance in getting onto the van for her program. PM : Help getting off the van, bathing, and having dinner after the program. On Sun, she remains at home and does not go out.
