About who needs care: Client is 90 yr old male, who has poor balance, memory issues, self care deficit, self regulation deficit. Goal is to keep him home and out of the hospital from sepsis caused by dehydration. He can appear perfectly normal and capable however, is not safe with decisions left to his own accord. About the care needs: Someone who is creative, engaged, and understands the in-between stages of end of life care. Some one who will push him and do things with him with the ability to verbalize and prevent him from doing something unsafe. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, and mobility assistance.
