Hands-on Care Needed For My Wife

We need someone to give me a break during the day and is able to take care of my wife while I take time off. She needs personal care, has mobility problems, problems with speaking, and sometimes dementia like problems. Help is needed to walk, sitting, feeding, and toileting. Other things to help would be nice, such as light housecleaning. Preferences for a caregiver who is a non-smoker, comfortable with pets. Current COVID vaccine would be a plus. Help is needed 2 or 3 afternoons 4 to 5 hours any weekday, sometime between 11:00 a.m. and 6:00 p.m. On the 4 hour days, I prefer the day to start at noon or later. If it is a 3 day week, then one of the days can be on the weekend.
