- Early onset, late stage Alzheimer's (67 y/o) - has a caregiver M-Fri 6A-4P, has 3 daughters who help at night/over the weekend - husband is working 6A-4 p.m. - has been increasingly behavioral with my father-trying to give him some respite. - still ambulatory with visual- perceptual deficits - occ incontinent, trouble finding the bathroom/orienting herself once she is in the bathroom - needs hands on assist for dressing - able to feed herself, requires set up/prompts - She enjoys watching sports, walking, her kids/grandkids; used to love gardening/keeping up her yard but hasn't been able to recently either Services needed include: meal preparation, assist to feed her dinner, night time ADL, taking her medications, putting her to bed, light housekeeping, companionship
