About who needs care: Heart failure, afib. She was previously independent, living at home by herself, but is deconditioned from a recent surgery and hospital stay. About the care needs: She is very weak and will be a total care. We also hope she will regain some of her ability to walk and transfer self. Services needed include: meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
