Hands-on Care Needed For Myself In Independence

About who needs care: wheelchair but can transfer with minimal assistance . moderately severe COPD: CHF; complex pain syndrome in lower right leg; low back issues, severe arthritis neck & right shoulder but cannot have more surgery. Semi-incontinent urine issues. Can feed myself and have small fridge & microwave but no other kitchen facilities. Living in converter basement garage with concrete floor that I fell on last March and broke my hip. currently bed-ridden due to severe muscle cramping from mid-back through low back and right hip and leg. Need help asap. About the care needs: don't really know. just general stuff. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
