About who needs care: Struggles with lower back pain and pain to walk & resides with his 78-year-old wife.
Uses a wheeled walker with seat around home, tries to be mobile without using walker. Cannot use stairs at this . time. Hobbies are coin collecting, gardening, working on computer, sitting on front porch (weather permitting). Gardening is not an option at this time. Struggles to get through PT. Some interests have been lost due to restricted mobility/pain. Patient needs positive support especially when he loses hope at times. Typically wakes around 8:30-9:00 a.m. and stays up for three to six hours depending on pain level. Naps in the afternoon when he tires. Sits and gets up out of chairs. Daughter preps meds and provides them at 9am, 10am, 4pm, 6 p.m. and 8 p.m. Patient eats small breakfast with meds at 10 a.m. with food. Bathes every 2 days in a walk-in bathtub independently (light supervision). Typically, eats light lunch 1-2 p.m. and dinner at 6-7 p.m. Overall, can move but experiences pain and patient has AFIB.
About the care needs: Personality -upbeat and positive with encouragement
Skills: Patience and keeping a watchful eye when walking to prevent falls. Patient can use toilet but wants to walk himself.
Hobbies: knowledge of talking about history (military was in Army) worked at DTE retired 27 years. He enjoys talking about where he worked.
Services needed include: transportation, feeding, bathing / dressing, and mobility assistance.
