About who needs care: Mary is 87 years old and getting ready to be discharges from a sub-acute rehab. She lives with her husband Frank, but he is currently in the sub-acute rehab with her and will not be discharged for a few more weeks. At that time they will also need assistance. She is walking well with her walker, but has a history of falls pre-walker. She does have mild dementia, but has managed taking her medications. She may need supervision with this task since new medications were added. She has controlled A-fib, HTN, hyperlipidemia. She loves to read and enjoys the outdoors. Her day would be help with meds and meals, light housekeeping, laundry 1X/wk. Possible transport to PT depending on d/c recommendations. About the care needs: She is very kind and simple, and a bit frugal. No special skills other than basic caregiver skills. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
