Job Description: Caregiver for Parkinson's Patient with Fall Risk Client Information: Diagnosis: Parkinson's Disease, Fall Risk Speech: Delayed (requires patience during communication) Mobility: Uses a walker for mobility Special Diet: Encourage gluten-free (GF) and sugar-free diet Allergies: Penicillin, Sulfa ________________________________________ Key Responsibilities: Daily Care and Assistance: Morning Routine (10:00 AM): Assist client with showering if needed (client can shower herself, but remain on standby to provide help as needed). Brush teeth and assist with dressing if required. Prepare and serve breakfast, ensuring any dietary restrictions (GF/sugar-free) are followed. Provide medication reminders and ensure proper administration. Mobility & Exercise Support: Assist with mobility using a walker, ensuring safety during movement to reduce fall risk. Drive client to appointments, primarily physical therapy (PT), on Mondays and Wednesdays. Encourage and support physical exercise, both during PT and at home, to help maintain mobility and strength. Offer social outings to the library or other places, and engage in games to help reduce isolation and maintain cognitive function. Light Housekeeping & Domestic Support: Perform light housekeeping tasks to maintain a safe and comfortable environment, including cleaning, organizing, and laundry. Change bed sheets and assist with any other light household chores as necessary. Health Management: Monitor client's health condition, including the impact of recent medication changes on bladder management and sleep patterns. Report any changes in condition to the agency or family, including improvements or concerns related to Parkinson's progression or fall risk. Client Communication and Relationship Building: Maintain a compassionate, patient, and supportive approach to communication, understanding the client's need for slower interaction due to speech delays. Develop and nurture a positive and trusting relationship with the client and their family, particularly focusing on building rapport with the daughter to ensure satisfaction with care. Ensure that the client's showering routine is followed as per the daughter's request (three times a week), regardless of the client's resistance. ________________________________________ Special Considerations: Ensure all care routines are adapted to the client's Parkinson's condition, maintaining a balance of independence and assistance where necessary. Understand the importance of patience and attentiveness when communicating with the client, especially when they are unable to speak quickly. Follow the dietary restrictions strictly to ensure the client's health is maintained.
