About who needs care: Has cognitive decline with dementia; forgetful and confused at times. Medical: Kidney disease, high BP, COPD, cognitive decline and dementia, vertigo, frequent UTI, fall risk Can ambulate with walker and assistance x1. Needs mechanical soft diet and assistance with eating and reminders to chew and swallow. Occasionally incontinent of bladder. Hard of hearing but has hearing aides. Poor eyesight due to macular degeneration. Likes to watch gameshow network and listen to gospel music. Family assists with care during the day and daughters stay nights. Additional help needed to stay for overnight care. About the care needs: An individual that understands dementia and is patient. Needs to be a CNA. No specific hobbies/interests but she enjoys having the Bible read to her and listening to Gospel music. Services needed include: feeding, and bathing / dressing.
