Client is a pleasant 81-year-old retired university professor with dementia. He can be hard of hearing, gets confused, has hallucinations (e. g responding to imaginary friends) and delusions (e. g. believes his mom who died in 2005 is visiting). 1. Keeping client safe by keeping client from wandering, ensuring door to the unit is locked. Specifically, be prepared to go for walks with client. 2. Engage client by reading Bible to him. 3. Go for walks with client as weather permits. 4. Administer medications in the AM & PM following the pillbox timing. 6. Client needs to be reminded to swallow multiple times. Have client open mouth to ensure medication has been swallowed. 7. Ensure all meds are taken. Client sometimes needs reminder to swallow meds. 8. Bathing and Toileting.
