About who needs care: Pt is dx with type 2 diabetes, congestive heart failure, chronic kidney disease and just had surgery in July 2024 for 4 fractured vertebrae that were fused together and suffered 18 broken ribs in a car accident on 7/5/24. He lost his daughter in the same accident. His hobbies are fishing, hunting and cars. We need someone with him during the weekdays(morning to late afternoon) to assist him with daily activities. Bathroom, bathing, meals, companionship and transportation to appts. About the care needs: He would like someone who is friendly. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, and mobility assistance.
