About who needs care: Recent fall, tib/fib fracture now post op. Currently in rehab but will be discharging home. Will need help with transfers, ADLs and/or overnight care depending on what my mom and aunts can help with during the day. Has baseline CHF, anxiety. About the care needs: Patient, she likes when people don't talk too fast as she has poor hearing. Services needed include: feeding, bathing / dressing, and mobility assistance.
