Live-in Home Care Needed For My Father In Evanston

We need a live-in caregiver for my father (74 yo, stroke victim, dementia). He is in a 1-bedroom apartment in Evanston, and I live in the apartment next door. He sleeps on the couch in the living room, so the bedroom is unused. The main responsibilities are medication reminders, preventing falls (he uses a walker), help using TV/electronics, and companionship. He mostly toilets independently, but sometimes needs help. It is a nice modern and clean apartment. We are thinking 5 days a week to take the load off me, and I'll do the other 2 days. There are no pets. Minimal cleaning as needed, but I will take care of keeping the place clean and stocked with any needed supplies. The majority of your time will be spent playing on phone/computer/etc or watching the TV in his bedroom (or with him) and keeping an eye on him. Schedule is negotiable.
