Hands-on Care Needed For My Grandmother In Saint Johns

I need a hands-on care provider experience for my grandmother in her 90s. She has old age dementia. She uses a Wheel Chair, needs max to total assist with transfers, able to take a few steps at time. She is incontinent at night, possible help with changes. Some of the responsibilities include helping with bathing, transfers, help with using commode and transfer into bed at night. She loves to be outside, so walks around the neighborhood with her in her Wheel Chair. Some personal care required, including feeding. Preferences for a caregiver who is a non-smoker and CPR / first-aid trained.
