Afternoon/evening Care For My Sweet 98 Year Old Dad

Looking for some afternoon/evening care for my 98-year-old father in West Simsbury. He walks with a walker independently but needs supervision. He is incontinent, needs help toileting and reminders to be use the bathroom. (No showering needed.) He needs some meal/snack prep but eats independently. He is a super sweet man and very compliant. He is hard of hearing, but wears an amplifier that works very well. He lives with my 92-year-old mother, who because of some moderate dementia, can't be in charge of his care, but is still helpful in knowing what he likes and keeping him comfortable. He doesn't require much else. He watches tv and reads. She does puzzles and plays cards.
