Seeking Transportation From Dialysis In Lone Tree To Home In Littleton 3days/week

Looking for transportation from dialysis center in Lone Tree to location in Littleton. My mother receives dialysis treatments on M-W-F. She gets out between 5 and 5:15(usually 5) and needs help getting to car. She walks but is unsteady and needs assistance. It is about a 20 minute ride to her house. She would need help getting into the house. Once home I am looking for someone to help her for a few minutes warming up a meal(no cooking), and perhaps doing a few things like switching laundry or getting something from the basement for her. Looking for someone reliable that can put an hours worth of time 3 days a week.
