Recuperative Care/Short Term Post Hospitalization Care

We are a non-profit organization that provides interim housing for Recuperative Care/Post Hospitalization Care for Partnership Health Plan members that are exiting hospitals, correctional facilities, mental health residential treatment facility, substance use disorder residential treatment facility, recovery residences. Our clients are individuals who are experiencing homelessness or those with unstable living situations who are too ill or frail to recover from an illness (physical or behavioral health) or injury in their usual living environment; but are not otherwise ill enough to be in a hospital. At a minimum, the service will include preparing meals and ongoing monitoring of the individual's ongoing medical or behavioral health condition (e. g., monitoring of vital signs, assessments, wound care, medication monitoring/dist., ADLs)
