About who needs care: Liver / kidney transplant in the 90s. 38 pills per day. Repeatedly falling and ending up in hospital. Needs to use walker but pain from falls makes walking difficult. On bad days, he needs help getting up and moving. Shaky hands makes self care very difficult. Incontinent and needs daily cleaning. About the care needs: Must be strong enough to lift him. Services needed include: feeding, bathing / dressing, and mobility assistance.
