Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, some minor help with feeding, bathing / dressing, toileting, and mobility assistance including total transfer to and from wheelchair (no hoyer lift). She has some leg strength but needs al lot of help and support with help with transfers to avoid falls. Occasionally can use a walker with assistance (being supported).
