About who needs care: He is a diabetic. 35 years ago he shattered his right ankle, requiring pins and plates. Then later had to have the hardware removed. During this time period of his life he went from 150 lbs. at 5' 6", to nearly 400 lbs. He was then diagnosed with type 2 diabetes and started on Metformin. Metformin made him extremely dizzy, causing him to have to pull off the side of the road until the dizziness was gone. Early one morning he got up to go to the bathroom, got dizzy, passed out and woke up when he hit the floor. BUT.... he fell on his good leg this time, on the right side, breaking his left heel, 2 places in that foot and 3 toes. On the inside of the left heel just above the ankle, a broken bone pushed through the skin. Now both ankles are weak and his knees are giving out. When he left the rehab in Pennsylvania, he weighed 316 lbs. About the care needs: Being able to help him up and down, showering assistant, help wash his hair and get the matting out. Teach him how to use the bottom wiping wand. Able to support his weight. Strength training, in-home physical therapy-type assistance, and probably OT to teach him how to give himself a bath/showering in his current condition. Needs to have a kind personality, but needs the ability to be firm on the bathing frequency and the exercises he can do on his own. Give me a list of things I can help him do to get to where he can hopefully help himself. They need to be able to motivate him to exercise as well as bathe, and to encourage him as he's exercising. Services needed include: feeding, bathing / dressing, and mobility assistance.
