Dialysis Transportation For My Mother

Looking for transportation for my mother to and from dialysis on Tuesdays, Thursday and Saturday's pickup from my house drop off at dialysis clinic 11:00 a.m. -12 p.m. pickup from dialysis clinic between 3:00 p.m. & 4 p.m. She has the on onset of dementia, walks with a walker most of the time and uses wheel chair when she goes to dialysis.
