Hands-on Care Needed For My Grandmother In Portland

About who needs care: She has limited mobility, so she is using a wheelchair most of the time. She also uses a urostomy, but is capable of maintaining it on her own, she may just need help getting to the bathroom (not using it, but she may need someone close just in case). Takes medicine. She needs to do physical therapy and walk at least a few times per day. She would like to have breakfast in the morning, sees her daughter (my aunt) for lunch and mostly watches TV or entertains herself most of the day. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, and bathing / dressing.
