About who needs care: Patient just returned home from Hospital after a week in the hospital and a week in rehab. Just need someone to keep an eye on him when he get up with walker. Patient is a fall risk caused by slight weakness to right side. About the care needs: This person has to be vigilant and be sure to keep and eye on him when he get up to use the bathroom. Patient is orientation. Just need help regaining ADL. Patient is able to feed himself, Move from chair to bed, walk to restroom etc. Patient might need help assistance with clothing . Services needed include: companionship.
