About who needs care: 91 yr old mother, currently at inpatient rehab post-back-surgery. Previously living in her own home independently-goal is to return to that status. Inactivity due to pain for a month prior to surgery has decreased her strength and endurance; she is improving in rehab, but not back to what she was. We need a bridge between inpatient rehab and home on her own. Light housekeeping, grocery shopping, laundry, rides to church (daily Mass at 8am), rides to appointments, and just to monitor her continuing improvement until she can manage on her own. Anticipating discharge within the next week; anticipate help needed for 2-4 weeks. About the care needs: Outgoing, personable- Mom loves to talk. Mom is fiercely independent, so the ability to support her independence while providing appropriate assistance. Services needed include: transportation, meal preparation, errands / shopping, and light housekeeping.
