Seeking Part-time Senior Care Provider In Wilsonville

I need senior care provider with experience. Our care concerns are: Dementia, Needs constant supervision, Mobility Assistance, Bathing / Dressing, Heavy Lifting, Companionship. My grandmother will be coming home to live the rest of her days on hospice. She will need to be changed, cleaned, and offered food, but not hand fed. She is in late stages of dementia and cannot get up on her own, so experience with transferring her to her chair and back is needed.
