The patient is a 64yo male who suffers from multiple psych disorders that is well balanced through an extensive medication regimen. Although most days are peaceful, there are times where he can need de escalation during outbursts. A caregiver who is patient, kind, and compassionate who possesses de escalation skills is a must. Although never violent, he can say things that may be offensive to the average ear. Patient often needs daily hygiene reminders including when to shower and change his clothing. Along with his medicated psych diagnosis, the patient also suffers from multiple physical comorbidities. He has multiple stents in his heart, as well as an artificial heart valve. He suffers from diet controlled diabetes and is a single leg below the knee amputee. Although mostly independent, he often needs help with bathing and balancing on his prosthetic leg especially during transfers. The patient is overweight, so weight/diet management is important. Because of his obesity, he also struggles with sleep apnea and wears a bipap machine every night before bed. He enjoys reading in his lazy boy and working on his many inventions as he is a 5x patented inventor. He is extremely intelligent and enjoys studying most of the day. Day to day job description includes general household chores (cooking, cleaning, laundry, taking out garbages), as well as making meals at specified times. Giving medications at specified times, and assisting with bathing, and changing clothes. Reading in his Lazy boy recliner as well as creating inventions. No certifications are needed. But preference for someone with CNA, or nursing experience. There is parking in the driveway, as well as street parking. The patient has their own van to use for transporting to and from appointments. No pets
